This is for a 20r overhead cam design; timing chain driven. So I think I might have a valve knock on the #4 cylinder (one closest to the cabin). How can a chain driven ohc engine have a valve knock on just one cylinder?

Could you describe valve knock ....that's a new term to me. Chain or belt....shouldn't matter based on the description you give.

If I'm not mistaken, most overhead cam engines also have valve lash adjustment in one form or another whether it be shims or adjustment screws.

You could also have a worn lobe on the cam.
